Ohio EMS Application

This application is required for admission into the Ohio EMS course. Additional requirements include the following to be submitted to:

• Must be age 17 or older. *If age 17, parental permission is required. 
• Must possess current driver’s license.
• High School diploma or equivalent
• Completion of a pre-entrance physical exam (prior to beginning of course) – [Link]
• Completion of a five-panel non-DOT drug screen.
• Full tuition or payment plan arranged or letter of intent from a sponsoring organization. See https://bit.ly/SSUOhioEMT for more information.
• Completion of an Ohio State & Federal background check. You may obtain from the SSU Department of Public Safety. Find out more HERE.
• A Criminal History Statement attesting to the fact that the student has not been charged with, pled guilty to or been convicted of the listed crimes. See page 3 HERE. 
• Conference with the Program Director
