Ohio EMS Application

All EMS programs will require the following additional documentation. These will be submitted at a later date in cooperation with the Center for Lifelong Learning.

• Must be age 18 or older
• Must provide a current driver’s license.
• High School transcript, GED or adult diploma.
• Completion of a pre-entrance physical exam prior to beginning of course.
• Completion of an Ohio State and Federal background check. You may obtain from the SSU Department of Public safety. Find out more HERE.
• A criminal History statement attesting to the fact that the student has not been charged with , pled guilty to or been convicted of the listed crimes. See page 3 HERE.
• Vaccine Record showing up to date tetanus, Hepatitis B, Varicella and MMR vaccines
• 2-Step TB test
• Interview with Program Director
• Letter of recommendation from a co-worker
